Registration and Emergency Information Form  Print, complete, and return. (Remember to provide all information; no skipping)	New and returning students One form per student
Information for School Directory Listing  Print, complete, and return.	New and returning students One form per family
Fundraising Event Participation Agreement Form  Print, complete, and return.	New and returning families One form per family
Travel Permission Form  Print, complete, and return	New students only One form per student
Field Trip Permission Form  Print, complete, and return	New students only One form per student
Documentation of Identity/Age  Print, complete, attach a copy of proof of identity (birth certificate or passport), and return. The copy of proof of identity will be returned to you.	New students only One form per student
Health Information Form/Comprehensive Physical Exam Report/Certification of Immunization  Print from Virginia Dept of Health website, complete, get physician signature, and return.	New students only One form per student Only required if one is not available for transfer from previous school.
